go to No. 83 Yen Ping north road section 2 . to get the application form at Counter no. 7 Tell them you want to apply for teh second employer working permit . Behind the form you will see which documents you have to prepare.

Go to the police station where you get your ARC to apply for the good resident certificate and the certificate proving that you have been staying in Taiwan for 5 years continuously. The form is at the counter . It will take one week after application and also 250 NT dollars.
Douments you need to prepare before you summit you application :

Application form With one passport size Photo

Copy of your Passport

Copy of your new working contract of your second company

Your new company 's owner’s ID copy ( need a stamp saying that it’s the same as the original copy ) （ 與正本相符 )
5.Copy of the second company’s business registration certificate

Copy of your previous working permit which proving that you have been working in taiwan for 5 years continuously.

Handling fee receipt 100NT dollars. ( this one you can do it at the application center . which is the post office located inside . Which is the right handside . There is a form for you to fill up and then pay 100NT . Tehy will give you a green receipt .

The Good resident certificate ( the one apply from police station. )

The certificate saying that you have been staying in Taiwan for 5 years . ( which you apply from the police station )

Copy of your current ARC
I guess this is all I can remember . It will take about a week to get the permit. They will send it your your new employer because you need a certificate if you want to pick it up by yourself . So it’s kind of troublesome.
After you get the new working permit . go to the police station again and let them ad the new company on your current ARC and this is free of charges.
