"Affidavit RE: Recognition of Marriage in Taiwan" necessary?

I’m an American citizen wanting to apply for a JFRV as soon as my health certificate gets in, and I’m wondering if I need this form (Affidavit RE: Recognition of Marriage in Taiwan") offered by the AIT. I understand that I need to supply proof that my marriage is “registered” in my home country, but that form itself states that registration of marriage is not required in America. I’ve also read that a marriage certificate can be provided instead of the proof of registration. So, should I get this form notarized by AIT, or is this just an attempt by them to obtain a sucker’s cash?

You no longer need it, at least as a Canadian who married in Taiwan and got my JFRV in Taichung, I did not need it.

THIS FORM IS NOT NEEDED.

I went to the AIT, got the form notarized, then went to BoCA to apply for a JFRV. The lady there looked at the affidavit in question, and handed it right back to me. I almost begged her to take it, so I didn’t waste my money, but it was no dice. A marriage certificate (and all the other required paperwork) is just fine.

sorry for helping AIT sucker you. it was needed when i got married.
