Does anyone have the Open Work Permit application form (.doc)?

I’m looking for a .doc file of the Open Work Permit application form. I had it (got it from Mr. Hartzell) but have long since lost it due to a hard disk failure.

Are there any of you out there who still have it? If so, could you email it to me? Would really appreciate it!

Thanks! I did take a look at the forms – seems they are for foreign spouses married to ROC citizens.

Do you have any for Permanent Residents?
