The values of some of the answers don’t go far enough, as others have mentioned above. If you want to do a survey, you might want to consider allowing others to review and comment upon the choices before putting the survey out.

Just a suggestion. For those who work at major chain buxibans, and perhaps the English teaching industry in general, there should be an option for question seven of “Prostitution”. Education & Training is not particularly accurate.
Seriously though, I wasn’t sure how to answer some questions adequately.
I’m fairly antisocial, so once a week doesn’t adequately describe my situation.
What is meant by a vacation? I go and stay with my in-laws once or twice a year, and I also go and stay at a friend’s apartment for a weekend once every couple of months, but you’d be hard-pressed to get me to part with some money for tourist accommodation in this country, being the stingy bastard that I am. So I’m not sure that I actually do vacation in Taiwan.
Are you talking about the amount income minus living expenses, or the actual amount that I spend on entertainment, buying toys, etc.? Big difference in my case as almost all of it goes into saving/investing. The reason I mention this is because I assume this survey has something to do with finding sponsors/advertisers for the site.

As The Chief never went to high school, and has at least two concubines, it’s understandable that he had problems answering some of the questions. Also, due to his background, it’s not possible for him to answer 13 consecutive questions without one of them being “is it time for another beer yet?”
I had no problems, although I lied about my age as usual. But having to choose between male and female was obviously difficult.
What will this survey be used for? It seems as though it will be used for marketing purposes because I’m not actually sure how the questions from this survey could actually be used to improve this website.
You’re right, we want to be able to better target advertisers, and it helps if we have the data that shows who looks at Forumosa, where they live, where do they come from, how much do they earn, etc. Believe me, advertising revenue will help improve the website!
Hence the option “never/rarely”. I think you’re covered.

Whether or not you want to add more options depends on you and your poll purpose, but I would suggest breaking Other down into ‘Central & South America’, ‘elsewhere in Asia’, ‘elsewhere in Europe’, and so on.
