How do you self report a positive PCR COVID test?

Hello. I called the CDC today and they finally offered me an alternative website in which to find my PCR test results. However, my PCR test was 4 days ago on the 12th, but no one has contacted at all. I have been in home quarantine since the 12th, but I still have yet to self report the test results.

I have a question. How do you self report a positive PCR test to the government?
