Sorry for the gravedig, but there still seems to be some confusion about this issue.
I’m pretty sure that the Council of Labor Affairs is the issuer of work permits for foreigners teaching English in cram schools.
Last night (or really, early this morning) I looked at six of my old foreign-English-teacher work permits–for 2004, 2005, 2006, 2007, 2008, and 2009. Each of the six had the following language, apparently in the nature of a letterhead, at the top of it:
(Xíngzhèng Yuàn Láogōng Wěiyuánhuì Hán; translation: Executive Yuan, Council of Labor Affairs (Letter?))

This is a translation of the pertinent part of Article 48 of the Employment Services Act:
