Obtaining "official" authorization to look up a relative's Household Registry Transcript

Hi,

I’m trying to recover a copy of my mother’s childhood Household Registry Transcript. I’ve identified which office in Taipei I need to go to, but have been told that I will need “official” authorization from my mother to look this information up on her behalf.

Does anybody know what “official” authorization looks like in practice? What will be accepted here in Taiwan?

Probably there’s a consent form to be signed, and I imagine you’d need some kind of copy of her ID.

Maybe someone here will know something definitive, but I’d just go to a household reg office and ask. There’s one in every district.
