Highly depends on the country. In some countries you might want to include a picture - in others probably not.
Is it OK for the CV to span more than one page?

Had an applicant a few years ago, their contact information included their email address, which contained “iluvcheese”…

Absolutely fine. Two pages tops though. No need for a photo, unless they request it (which is probably illegal under most EU laws) CV = Resume. I wouldn’t use a silly email address, personally.
