APRC, filing for long-term absence without a TW address?

I had to move back home to take care of my elderly mother. Before I left I filed for long-term absence and it was approved, but at that time I still had a Taiwan address. Now I don’t, since I moved out of my apartment and put my stuff in storage.

Do I still fill in my old address on the form, do I use my U.S. address, or do I leave it all blank? Any other issues I need to know about to renew my absence for another two years so that I don’t lose my APRC?
