If I’m not wrong, the work permit for you is supposed to be applied by your employer(s). At least, you should get a help from your employer, if they want you fill out some parts by yourself.

The trouble is that you cannot understand what to do on the page, so the government agency can do nothing for you. You should ask for a help or tell to do it by themselves to your employer.

I can’t remember the entire procedure but in general your employer should do this. I’m assuming you’ve taken a teaching job in which case they will take copies of your passport and degree and then take you for a health check. Then they will process it. I’m not sure what’s going on here, but it seems very odd.
