Please help!
I am an American citizen who lived in Taiwan for a short time in June to July of 2010. I am in the process of adopting a child from a Hague Convention country in Africa. As part of this process, I need to provide a child abuse check for anywhere I have lived for more than three weeks… which includes Taiwan!

The problem is the page talks about an “application” to mail, but I do not see one anywhere on there. Can anyone help me figure out what application this is?
