So much has changed – we are using newer versions of phpBB, different server hardware, newer versions of phpBB mods. You could say that EVERYTHING has changed and not be far off from the truth. And apparently, the ignore mod itself has changed (my evidence? the Moderators Cannot Be Ignore feature seems new to me)

I think this is the New Posts/Ignore list conflict bug
I had the same problem but when I cleared my ignore list everything works fine…
