I need to get a US Marriage certificate notarized (as a Taiwan citizen) at TECO for my marriage to a PRC citizen. Looking for some advice about details.

I have the option of getting just the certificate notarized, or also notarizing the long form application form with a lot more personal details. Which one is recommended?
