Health insurance companies don’t get rich by denying payouts. As the economics blogger Noah Smith points out, UnitedHealthcare’s net profit is about half of the average of S&P 500 companies. According to the Harvard economist David Cutler, who has written extensively about the US healthcare system, healthcare costs are so high because of administrative inefficiencies. Insurance companies have become so bureaucratically bloated as to administrate a wildly unstandardised healthcare system. This bloat now accounts for one-third of the delta between US healthcare costs and those of other high-income countries.

Huh, guess he should have shot the bloat. Imagine that-- the nutter was wrong.

Since 2020 every US doctor I’ve met in private practice complains about regulatory burden. They all say good luck to new doctors who want to set up their own practice. The key to success, they say, is hiring at least one back-office expert in health insurance. My PCP told me he joined a small physician group for just this reason and that they have ten (10!) non-medical employees who are experts in insurance remuneration, all on salary and each earning >$US100k/yr. That’s >$1M in salary expense before the docs themselves are paid their first dollar. There are four doctors in that group.

At the federal level DOGE would probably be a huge winner - would put $$ in every American’s pocket - if it could just find a way to streamline federal healthcare regulation.
