I actually don’t like the Mcdonalds in Italy very much. It doesn’t taste right. I can’t figure out why it taste different. It’s not bad, but just doesn’t taste like how I know the food in McDonald taste. I guess the local ingredients is different?
If either of the plain old hamburger (the tiny one) or the plain old cheeseburger is on the dollar menu, then I always get that.
The quarter-pounder is ok now and then, so is the Big Mac, I like the fish burger, but tbh the best sandwich at McD is still the original.
Those tiny burgers are totally underrated in my book.

I sometimes take half of the hamburger and smoosh it with another half of the spicy chicken burger. 
And maybe layer some fries in there.
Damn. And here I didn’t want to eat until later. 
Look at how clean the local restaurants are, then mcdonalds is like heaven here.

I don’t know about the menu in Taiwan but hk’s is neverending change.
There are numerous choices on burgers here. Google it!
I love the different seasonal pies such as corn, taro, red bean, red bean and custard, yam, pineapple, peach etc etc
I’m talking about the basic hamburger in the US (I rarely eat at McD here in Taiwan).
I was never a big fan of the fried pies, because Hot Pockets.

Hk’s are great with speed and efficiency plus high quality food and clean dining rooms.
McCafé has excellent New York cheesecake toasties muffins etc at reasonable prices.
I don’t dine there regularly btw.

HK mcdicks? I found them only slightly better than north American ones. But I only sampled two stores. Taiwan’s wins hands down IMO.

There was a leftover Whopper in the fridge this morning from my kid’s Club Night.
I was in a rush so I miked it up and took it for breakfast.
I tasted so good I’m thinking there might be something seriously wrong with my nervous system.
regarding OP – this has been my experience only in the US… but when I go to Taiwan mcdonalds everyone is pretty normal – even athletic appearance, at least in Taipei. late night you even see lots of well dressed business people or shirt/tie people dining in at the mcdonalds. I guess because in TW they only give you military ration levels of ketchup whereas in US would probably get 4 to 6 small packets or just be able to grab them yourself. also for the record, Taiwan McDonalds is best quality/consistency by a mile compared to any others – maybe Singapore would be at same level.
